Not quite what I want - the categories are created dynamically from a db. It 
seems such a simple thing - and it is possible to 
have /index/category/item - or even /i/category/item - but it seems stupid to 
me that you can't get rid of the name of the page that process the path info. 
I have read just about every article there is on url-rewriting, etc. I think. 
The only semi-viable solution so far is custom 404 processing - and that has
other problems.
