Less?  If they die sooner, the ratio of schooling / work is worse.  
Therefore their payment of taxes and health insurance is much lower.  
And their health costs are much higher all along.  Both the chronic care 
of diabetes and the major medical malfunctions they tend to have 
(diabetes end stages, heart disease, cancer, breathing problems), and 
extremely expensive care of these totally blows out their total medical 
costs.  A non-smoking runner might have less medical costs in 40 years 
than an unhealthy person has in 1.
