Been watching too much Fox News? The only solution we've got right
now is to have independent (e.g. government, but perhaps not
necessarily) regulation that monitors failures of redundant systems.
If you fine 'em when one of their three redundant systems fails, the
probability that they will run with only one system operational, and
then blow up is dramatically reduced. Obviously the proper
inspections weren't done on DWH. The NRC though does do this for nuke
plants, and has been fairly successful so far. The problem is that
the penalty for failure only goes up as the energy levels go up.
